No-code-change comment: pipeline wiring & resume

Steps 4–6 of the no-code-change comment feature (ADR-021). Wire the deterministic post into runPipeline's no-code-change short-circuit with fatal write-back attribution, surface the posted comment in the run summary + CLI outcome line, and teach resume that a valid no-code-change.json means implement is complete (resume at review, never re-running the implement LLM).
