No-code-change comment: verification & docs

New issue

Steps 7–9 of the no-code-change comment feature (ADR-021). Consolidated unit-test seams (seam→test-file matrix, npm test + lint green), the gated resume E2E (PILOOP_E2E=1) proving a fatal comment failure resumes to exactly one posted comment, and the AGENTS.md + DESIGN.md doc updates with the full validation sequence.
